Appendices to the notice of injury

You need not attach any medical records with your notice of injury. The Patient Insurance Centre will obtain this information directly from your place of treatment. Do not attach any prescriptions, receipts or sickness allowance or pension decisions with your notice of injury, unless they are of relevance for assessing the appropriateness of the treatment. Such clarifications will usually be requested during the claims handling process if the Patient Insurance Centre decides that the injury you reported is a compensable patient injury.

If you need to submit original material, e.g. plaster models or imaging materials, they must be sent to the postal address of the Patient Insurance Centre, PO Box 1, FI-00084 VAKUUTUSKESKUS. You can also deliver material as a registered letter. As the sender of the letter, you will be responsible for the postage fee. We will add internal markings to the original material you send to identify the material.

Retain all invoices and receipts so that they you can later include them with your claim for compensation where necessary. If you also are claiming compensation for the expenses elsewhere, for example from the Social Insurance Institution of Finland (Kela), make copies of the documents. Also save any claims decisions issued by other parties when you receive them.
