Forms for filling a notice of patient injury

Use the notice of injury form provided by the Patient Insurance Centre to file a notice of the injury. You can also file a notice of a patient injury online.

You can claim compensation under patient insurance by filing a written notice of injury. Instructions for filing a notice on injury are available on our Filing a notice of injury page.

Before filling out the notice of injury, it is advisable to take the online test provided by the Patient Insurance Centre.

Form for filing a notice of a patient injury

You can file a notice of a patient injury with the Patient Insurance Centre in three different ways:

  1. by submitting an electronic notice of injury in the online service
  2. by filling out an online pdf form. Sign the form and send it to the Patient Insurance Centre by post.
  3. by filling out the hardcopy form. Sign the form and send it to the Patient Insurance Centre by post.

Electronic notice of a patient injury

You may file an electronic notice of a patient injury when:

  • you have online banking credentials available. You can fill out and submit the electronic form once you have authenticated with our online service
  • you are filing a notice of injury concerning your own injury case
  • you are filing a notice of injury concerning the injury case of your underage dependent
  • you are filing a notice of injury concerning the injury case of an adult and legally incompetent person and you are his or her officially appointed trustee. Enclose the guardianship order issued by a local register office or a court of law with the notice of injury
  • you are filing a notice of injury concerning a case of deceased person and you are a party to the estate of the deceased or another next-of-kind who has incurred financial loss or damage as a result of the patient's death.

It is advisable to file an electronic notice of injury between 6 a.m. and 10 p.m., because there may be maintenance breaks in the system of the Patient Insurance Centre during the night.

Fill out an electronic notice of injury in the online service on your own computer and then submit it to the Patient Insurance Centre. Check the information in your notice before submitting it.

You will receive a confirmation on your own computer when your notice of injury has been successfully received by the Patient Insurance Centre. After submitting the form, you can save or print out your notice of injury.

Pdf form for filing a notice of a patient injury

On the Patient Insurance Centre website has been published an online form for filing a notice of a patient injury (pdf).

Use the pdf form for filing a notice of a patient injury when:

  • you do not have online banking credentials available
  • you wish to authorise another person to attend to the processing of your claim at the Patient Insurance Centre.

You can fill out the form and print it out on paper. The pdf form cannot be sent electronically as an e-mail attachment, for example. The form must always be duly signed and sent by post to the Patient Insurance Centre. The postal address of the Patient Insurance Centre is Potilasvakuutuskeskus, 00084 POTILASVAKUUTUSKESKUS.

If you are unable to print out the pdf form, order a hardcopy form from the Patient Insurance Centre by e-mail at lomaketilaus(@)vakuutuskeskus.fi or by calling us at +358 40 450 4590 (from 12 a.m. to 3 p.m.). You can also ask the Patient Ombudsman or a social worker of your place of treatment if they have any hardcopy forms available.

If the patient injury occurred before 1 May 1999, use the special form intended for reporting them (in Finnish). You cannot file a notice of such patient injuries via our electronic service.

Documents to be attached with the notice of injury

You need not attach any medical records with your notice of injury. The Patient Insurance Centre will obtain this information directly from your place of treatment.

If you need to submit original material, e.g. plaster models or imaging materials, they must be sent to the postal address of the Patient Insurance Centre.

Do not enclose any prescriptions, receipts or sickness allowance or pension decisions with your notice of injury, unless they are of relevance for assessing the appropriateness of the treatment. Such clarifications will usually be requested during the processing at the claims handling stage if the Patient Insurance Centre decides that the injury you reported is a compensable patient injury.

Powers of attorney for attending to a patient injury case

If you wish to authorise another person to attend to the patient injury case on your behalf, you may enclose your duly signed power of attorney for attending to your patient injury case (in Finnish). The power of attorney may also be delivered as an enclosure at a later stage when the notice of injury has already been filed with the Patient Insurance Centre.

If you filled out the authorisation section in the pdf form and thus authorised another person to attend to your case, you will not need to send a separate power of attorney.

If you file a notice of a patient injury electronically, you cannot authorise another person to attend to your case on your behalf at the Patient Insurance Centre.

The Patient Insurance Centre must always be notified of the cessation of the authorisation in writing.
If the patient has deceased, the parties to the estate can authorise one person to attend to the patient injury case and to receive the compensation. To grant such authorisation, the power of attorney for estates (in Finnish) must be filled out and duly signed.
